BENEFITS
S&H Machine offers a comprehensive
benefit package that is unprecedented for a small business. The following
benefits are for all covered / eligible Employees of S&H Machine:

HEALTH INSURANCE:
Paid 100% HMO by S&H Machine for the employee. (After 1 month of employment) Employees have the
option to pay the difference to obtain coverage for their family members or
PPO.

GROUP LIFE INSURACE: $15,000.00 paid by S&H Machine. (After 1 month of employment)
á
ADDITIONAL LIFE INSURANCE: Paid by employee at a competitive group rate.
(After 1 month of employment)

DISABILITY INSURANCE: 50% paid by S&H Machine and 50% paid by the employee. (After 1
month of employment)

DENTAL:
Employees receive $275.00 in dental care per year for employee and family paid by S&H Machine. (After 1 month of
employment)

401k: S&H
Machine matches .50 cents for every dollar the employee contributes (up to 6%
of annual pay). Funds can be placed in cash, stock, or bond funds of employeeÕs
choosing. (After 1 year of employment)

PROFIT SHARING:
Each year a contribution is made by the company into the employeeÕs retirement
fund based on the profits of the company. Funds can be placed in cash, stock,
or bond funds of employeeÕs choosing. (After 1 year of employment)

VACATION: 2
Weeks Vacation is paid to each employee after 1 year of employment and 3 Weeks
Vacation after 5 years.

SICK PAY:
Employees are paid 6 sick days per year.

HOLIDAY PAY:
Employees are paid for 7 major holidays each year.

PERFORMANCE COMPENSATION BENEFIT (PCB): PCB is a performance based compensation
program that is designed to reward employees for better than average work.
It is a reward for the top performing employees. PCB is in addition to
our existing benefits and bonuses. The PCB program allows the employees
that are working hard to earn more money. Employees are graded monthly based on
a ÒPerformance TriangleÓ which includes Quality, Quantity and Attitude.

VISION: Full time employees are covered for annual eye exams ($50.00
Maximum) and Prescription Glasses (Up to $150.00) per year.
á
BONUSES: Bonuses may be given at anytime and at the discretion of
management; bonuses may be based on merit, length of service, position, and /
or company profits. Currently all
employees receive bonuses twice per year: once in June and once in December
á
EMPLOYEE OF THE MONTH / EMPLOYEE OF THE YEAR: Each month all S&H employees submit
a vote to elect the Employee of the Month. Typically this award is given to the employee who best
exemplifies outstanding teamwork, exceptional detail to his or her area of
responsibility and whose actions help to create value for the company. The benefit for receiving this award is
an extra dayÕs pay on the employeeÕs paycheck at his or her current rate of
pay. At the end of the year the
Management Team reviews the twelve ÒEmployee of the MonthÓ recipients and
elects one employee from this pool as the Employee of the Year. The benefit for receiving this award is
an additional weekÕs pay on the employeeÕs paycheck at his or her current rate
of pay.
TERMS, ELIGIBILITY AND
DEFINITIONS:
The Corporation reserves the right to
alter, amend, modify, suspend or terminate this Plan at any time and in any
respect. ÓCovered / Eligible EmployeesÓ shall mean all full time employees who
have been with the corporation for more than one year (for the 401k / Profit
Sharing Plan) or more than one month (for all other benefits listed above) as
of the start of a calendar year. A Òfull time employeeÓ shall be defined as an
employee who regularly works more than 25 hours per pay period.
